Student Referral form to meet with KMS Career Coach Ms. Ford

Know a student who could use support with career planning or future decisions? Students, parents, or staff can all submit a referral form (one per student), in addition to the QR code hanging on Ms. Ford's office door, right across from the cafeteria. 

Ms. Ford can meet with students for up to 20 minutes at a time, so please include a few sentences about what support would be most helpful (career exploration, decision-making, next steps, etc.). Referrals are reviewed at least biweekly. Thank you for helping connect students to support!
